چکیده

DeBakey and Simeone (1946) found that 686 per cent of amputations among United States troops in the second world war were done as the result of arterial trauma. Amputation was required in 4O 3 per cent of cases of arterial trauma. Arterial injuries associated with fractures showed a 50 per cent loss of limb with reparative measures, whereas there was a 70 per cent loss of limb when the vessel was ligated. During the Korean War more advanced arterial techniques were used, particularly in reconstruction, with consequent improvement in the results-the amputation rate after arterial repair was reduced to 7 per cent (Morton, Southgate and Deweese 1966). This paper reviews twelve patients with major arterial trauma-six closed injuries and six open-admitted to the Surgical Professorial Unit of the Richmond Hospital, Dublin, from 1964 to 1966.

Initial clinical assessment of non-traumatic acute limb ischaemia: A high degree of suspicion is pivotal in timely intervention

Acute limb ischaemia is both a limb-threatening and life-threatening surgical emergency. The clinical presentation of acute limb ischaemia can vary depending on the underlying aetiology as well as pathophysiology. While traumatic acute limb ischaemia becomes obvious in a trauma victim, nontraumatic ischaemia can be easily mistaken for other neurological or medical conditions. The rapidity with ...

Acute Type A Dissection and Acute Inferior MI Complicating Aortic Coarctation: A Case Report

Aortic coarctation is a rare but dangerous condition. Presence of aortic dissection in addition to coarctation is significantly rarer and more life-threatening. Here, we present a case of acute inferior MI due to aortic dissection complicating a native aortic coarctation. A 17-year-old boy was admitted to our emergency department due to severe chest pain and loss of consciousness. His ele...

Reperfusion strategies in the management of extremity vascular injury with ischaemia.

BACKGROUND Extremity injury with ischaemia is the most common pattern of vascular trauma and is a challenge for surgeons who must make decisions about the timing and mechanism of limb reperfusion. In modern military conflicts, effective use of limb tourniquets and rapid transport of the injured have increased the number of casualties who reach a medical service with potentially survivable vascu...

Management Strategy in Non-Limb-Threatening Acute Ischaemia of Limbs: Should We Rethink?

The Society of Vascular Surgery and the International Society of Cardiovascular Surgery identify three types of acute limb ischaemia to inform prognosis and management. Type 1 limb ischaemia is non-limb-threatening and is currently managed conservatively. We describe three cases of Type 1 limb ischaemia with femoropopliteal occlusion that were managed differently. The first case was initially m...

Application of the 64-slice computed tomography as a diagnostic method in acute posttraumatic ischaemia of the upper limbs – 3 case reports

BACKGROUND Acute limb ischaemia' is a sudden, rapidly progressing inhibition of blood supply to a limb, characterised by appearance of new symptoms or by aggravation of the already existing ones, which may lead to amputation of the limb. Computed tomography angiography (CTA) performed with a multislice scanner belongs to methods used for arterial imaging in acute limb ischaemia. The main advant...
